Five parts, in the order they appear on the certificate itself. Everything after Part 1 is copied from the contract or the holder’s written requirement — if you have that paperwork in front of you, this takes about four minutes.

The named insured is the entity on the policy, which is not always the person filling this in.

02 Who the certificate goes to

The certificate holder. This block prints on the form, so spell it the way their contract spells it — a general contractor will reject a certificate made out to the wrong entity.

The first three are endorsements to the policy, not lines of text on a certificate. If your policy does not already carry them, we have to go to the carrier before anything can be issued — so tick them here rather than after the certificate is rejected.

Do not attach the contract here. There is no upload box on this page on purpose — contracts and insurance requirements carry policy numbers, license numbers and sometimes payment detail, and this is ordinary web mail rather than a secure portal. Sending this is not issuance of a certificate. It reaches the Houston desk as a request. A certificate is issued only after we have checked it against the policy in force, and nothing on this page binds, changes or renews coverage.

    What the holder is asking for is compared against what you actually carry. Most requests clear here.

    If an endorsement is missing, we call

    Additional insured and waiver of subrogation have to exist on the policy before they can appear on paper. That is a carrier request, and it is the step that takes time.

  4. 04

    The certificate goes where you said

    Issued from the agency system and sent to the holder, to you, or both — whichever you picked in Part 5.
